New Clients

New clients and returning clients who have been away for more than 90 days, will need to complete an intake form - preferably online (and sign other documents later), and the forms will be sent to me directly. If the packet is not completed prior to your appointment, you will need to complete it when we meet for your intake session. If you take more than 15 minutes into your session time to complete the intake, the appointment will need to be rescheduled. 

**New client appointments are typically not scheduled for Mondays. If your initial appointment is not reconfirmed at least one business day before the scheduled time, it may be administratively canceled. Please be courteous and appear for your appointment or cancel it before the scheduled time.** 

If you are seeking couples or family counseling, it is requested that the primary person as well as other family members who are 13 years of age or older, complete the electronic intake form and submit it within the portal prior to the appointment. Services for couples and/or family counseling are treated as one client with the rate identified for each service. Please feel free to contact me with any questions or concerns. Client appointments can be requested, by calling the practice office directly. All appointment requests need to be approved before they are confirmed.
